Our comprehensive survey evaluates your life and goals across ten dimensions, ranging from relationships to finances to health. Developed by licensed mental health professional and founder of Thriveworks, AJ Centore, PhD, the survey also identifies what you’re seeking from a forum—whether it’s social relationships, recreational experiences, or support/guidance in specific life or professional areas.

Survey Details:

• Duration: Approximately 10 minutes
• Process: Simple and straightforward
• Confidentiality: Your responses are held in strict confidence and are NOT shared with prospective forums. Your responses are NOT shared with general staff at RF. Currently, only Dr. AJ Centore reviews and scores responses.

Assurances:

• You are encouraged to answer honestly and completely.
• If any question is uncomfortable, you may skip it. However, the more information you provide, the better we can match you with your ‘ideal forum.’
• The survey has not been scientifically validated or normed, but this may be considered in the future.

Fee Note:
